This referral form does not give Insightful Impact consent to create a service booking, nor does it give us access to the participant’s NDIS funding.
On receiving new referrals, allocations are made via the Clinical Governance Team as Practitioner capacity allows. Please note that to ensure the wellbeing of our team and the quality of our service, Insightful Impact does maintain a waitlist.
Plan information is sought solely to support Insightful Impact in understanding your service needs and to allocate the most suitable practitioner to support you.
At no time are you obliged to provide a copy of your NDIS plan to any provider if you choose not to.

What is the NDIS?

There are around 4.3 million Australians who have a disability. Within the next five years the NDIS will provide an estimated 500,000 Australians who have permanent and significant disability with funding for supports and services. For many people, it will be the first time they receive the disability support they need.
